Most people have heard of leaky gut at this point. Fewer people understand what it actually means or why it matters so much for overall health.

Research into intestinal permeability has grown significantly over the past decade. While the term “leaky gut” is commonly used by consumers, researchers typically refer to increased intestinal permeability when discussing changes in gut barrier function. (1) It is a physiological process that scientists are actively studying, and its potential connections to broader health are becoming increasingly difficult to ignore.

If you have been dealing with persistent bloating, fatigue, food sensitivities, or a general sense that something is off with your digestion, it may be worth understanding what is happening at the gut wall level and which supplements have the most evidence behind them.

What Leaky Gut Actually Means

Your intestinal lining is designed to do two opposing jobs at the same time: absorb nutrients and block harmful substances. It manages this through structures called tight junctions, which are essentially microscopic seals between cells in the gut wall.

When those tight junctions are working properly, nutrients pass through into the bloodstream and everything else stays where it belongs. When they become weakened or damaged, the barrier loses its selectivity. Bacteria, undigested food particles, and inflammatory compounds can move through the gut wall and into the bloodstream.

Beneficial bacteria help maintain the mucus layer, produce compounds that support tight junction proteins, and reduce inflammatory signals that can damage the epithelial barrier. (5)

A 2023 pilot study published in Digestive Diseases found that a multistrain probiotic may reduce intestinal permeability in a meaningful proportion of IBS-D patients, with some participants also reporting improvements in abdominal pain and quality of life. (6)

One important consideration with probiotic supplements is survivability. A probiotic that does not survive the acidic environment of the stomach will not deliver meaningful benefits to the gut. When evaluating a probiotic, the delivery mechanism and strain stability matter as much as the CFU count.

Digestive Enzymes

Digestive enzymes are proteins produced naturally in the gastrointestinal tract. Their job is to break food down into forms small enough for the body to absorb and use. Lipase handles fats, amylase handles carbohydrates, and protease handles proteins.

When enzyme production is insufficient, food can remain partially undigested. In some situations, poorly digested food may contribute to fermentation and digestive symptoms such as gas and bloating, which can add to the inflammatory burden on the gut lining. (5)

A 2023 randomized, double-blind, placebo-controlled trial published in Biomedicine and Pharmacotherapy found that digestive enzyme supplementation significantly reduced symptoms in people with functional dyspepsia compared to placebo. (7)

For people who notice that specific foods consistently cause bloating or discomfort, inadequate enzyme production is worth considering as a contributing factor.

Collagen

Collagen is commonly promoted for gut health because it provides amino acids involved in connective tissue maintenance, including glycine, proline, and glutamine. Type I and Type III collagen are found in the extracellular matrix that supports the epithelial lining of the gut wall. (8)

Glycine in particular has anti-inflammatory properties and may play a role in tight junction function. Some in vitro research has shown that collagen peptides can help reduce intestinal barrier dysfunction in cell studies, though direct evidence for improving intestinal permeability in humans remains limited. One small human study found that daily collagen peptide supplementation over eight weeks was associated with reduced bloating. More rigorous research is needed before stronger conclusions can be drawn. For people already taking a broad gut health approach, collagen may offer supportive amino acids, but it should be understood as one piece of the picture rather than a primary intervention.

A Few Honest Notes on Expectations

Supplements work best as part of a broader approach. For most people, meaningful gut healing involves identifying and reducing the underlying triggers, improving diet quality, managing stress, and giving the body time.

There is no supplement that overrides a high-sugar diet, chronic antibiotic use, or ongoing stress. But for someone who has already made foundational changes and wants additional support, the categories above have the most evidence behind them.

Results also vary considerably between individuals. Gut health is deeply personal and influenced by a wide range of factors including genetics, microbiome composition, and health history. What helps one person noticeably may not produce the same results in another.

Putting It Together

Leaky gut is not a permanent condition for most people. The gut lining has a real capacity for repair when given the right inputs.
