Milk Thistle for Liver Health: Benefits, Research, and How It Works

Most people don’t give their liver much thought until something goes wrong. But for an organ that quietly manages over 500 functions every day, it takes on a remarkable amount of stress. And one of the oldest herbal remedies in recorded history has been getting renewed attention for how it may help.

What Is Milk Thistle?

Milk thistle (Silybum marianum) is a flowering plant originally native to the Mediterranean region. Its seeds contain a group of active compounds called silymarin, a mixture of flavonolignans that give milk thistle most of its therapeutic properties.

Silymarin has been studied for its antioxidant, anti-inflammatory, and antifibrotic effects, meaning it may help protect liver tissue from damage and reduce the kind of internal scarring that can develop with chronic liver stress. (1)

One of silymarin’s most researched properties is its ability to act as an antioxidant and anti-inflammatory compound inside liver tissue. A 2024 review published in PMC found that multiple clinical studies affirm that silymarin can positively influence liver disease progression, reduce elevated liver enzymes, and support overall liver function in people with chronic liver conditions. It may work against liver scarring

Silymarin has demonstrated antifibrotic properties in research, meaning it may help slow or reduce the internal scarring (fibrosis) that can develop when the liver is chronically inflamed. Fibrosis, if left unchecked, can progress to cirrhosis, a more serious and irreversible stage of liver damage. (3)

It may help lower liver enzyme levels

Elevated liver enzymes (ALT and AST) are often a sign that the liver is under stress. A systematic review published in Cureus in 2023 analyzed 29 randomized controlled trials involving nearly 4,000 participants and found that silymarin supplementation was associated with improved liver enzyme levels across a range of conditions. Results varied by dose and population, and the authors noted that more research is still needed to standardize recommendations. But across the body of evidence, the direction is consistently supportive.

Beyond the Liver: Other Areas of Research

Milk thistle’s benefits don’t stop with the liver. Researchers have explored its potential in several other areas, though it’s worth noting that some of this evidence is earlier-stage.

Blood Sugar and Metabolic Health

There’s a meaningful connection between liver health and blood sugar regulation. The liver plays a central role in regulating blood glucose and responding to insulin, so when liver function is compromised, blood sugar balance often suffers too.

A meta-analysis examining silymarin’s effect on type 2 diabetes found that supplementation was associated with reductions in fasting blood glucose and HbA1c. (5) The authors were careful to note that the quality of available evidence was limited and that further studies are needed before firm conclusions can be drawn.

Brain Health

A growing area of interest is silymarin’s potential neuroprotective properties. Some animal studies have suggested it may help protect brain cells against certain types of stress and inflammation associated with aging. This research is early-stage, and human trials are limited. It’s worth following, but firm conclusions aren’t yet possible.

Milk Thistle Dosage: What Research Suggests

Dosage recommendations for milk thistle vary depending on the intended use, and there is no universally agreed-upon standard. Most clinical studies have used silymarin doses ranging from 140 mg to 600 mg daily, often divided into two or three doses. (6)

The NIH’s StatPearls reference notes that patients in one clinical trial who took 600 mg of silymarin daily for 12 months showed meaningful reductions in insulin resistance. Other studies have used lower doses, around 210 mg to 420 mg daily, with positive results in liver enzyme levels. (6)

For general liver support, many practitioners and researchers suggest starting on the lower end of that range. As always, the right dose for any individual depends on their health status, the reason they’re taking it, and any other medications or conditions they may have.

  • Standardized silymarin content: Look for supplements that specify the percentage of silymarin in the extract. Products standardized to 70% to 80% silymarin are most common in research.
  • Dose transparency: The label should clearly state how much silymarin is in each serving, not just the total herb weight.
  • Form: Milk thistle is available as capsules, tablets, liquid extracts, and teas. Teas tend to deliver significantly less silymarin per serving than concentrated extracts.
  • Quality assurance: Choose supplements from manufacturers that follow Good Manufacturing Practices (GMP) or undergo independent third-party quality testing.

A Few Things Worth Knowing

Milk thistle is generally considered well-tolerated. Side effects are uncommon at recommended doses, though some people experience mild digestive upset. Very high doses may cause laxative effects.

Because silymarin is processed by the liver, it can potentially interact with medications that are also metabolized there. This includes some cholesterol medications, certain diabetes drugs, and others. Always check with your doctor if you are on prescription medications.
